目前对于中国大陆的申请人来说，美国临床医学的申请仍是禁区。笔者从业美国留学多年，目睹了太多有志于去美国攻读临床医科的青年才俊被美国医学院严苛刻的入学标准拒之门外。为此，笔者多方面收集美国医科教育资料，力求能为有志于美国医科教育的后来者们提供一些借鉴。临床医学虽是禁区，但希望本文能为他们开辟一条曲径小路，以完成在美国求学医科并成为职业医师的梦想。
在美国要考行医执照首先要必须的条件是1.MD，2.绿卡。对于本身是临床的学生，国内临床医学的本科学历相当于美国医学院MD教育，在考行医执照的时候是被认可的。
如果本身在国内并不是学临床医学的学生，想要申请到美国医学院太难了。即使申请成功，MD几乎不给国际学生奖学金，高额的学费也让很多人望而却步。至于绿卡，也是必要的条件。如果没有绿卡，即使是考取了行医执照，也很难找到工作。
没有绿卡的话，持J1签证或者H1B的MATCH也可以，但是要求拿到行医执照后回国工作两年，两年之后能否获得美国签证很难保证。所以对于国内临床的学生来讲，想到美国从事医师的合理路径为：申请基础性学科的PHD――5.6年毕业之后做post-doctor-拿到绿卡（或者找工作H1B身份顺利的话3年之后可以拿到绿卡，但是现在办理时间呈现越来越长的趋势，在这期间你不能更换工作，否则年数作废）之后你便可以复习考取行医执照了。
美国的行医执照考试叫USMLE（US MEDICAL LESENSURE EXAMINATIONS），由三部分组成：
STEP 1是考基础， 生理， 生化， 病理， 解剖等。 考试考8个小时，涉及大概11-12门医学基础课。
STEP 2考试考临床，考9个小时，涉及内外妇儿神，精神，预防，耳鼻喉，眼科等8-9门 临床课内容。
CSA是临床技能评估（CLINICAL SKILL ASSESSMENT），考1天，有10-12个考官假扮病人，每个SP，STANDARD PATIENT，只给你25分钟，7分钟问诊，8分钟查体，10分钟写出诊断和病例，考的是你的临床技巧和思维判断。对于中国人来说明显处于劣势，印度或英国等国家的毕业学生都是用英语上课，英语熟练程度要高。中国学生就建议用英语的角度把所有基础和临床课都自学一遍。
现在STEP2和CSA考试已经合并了，成了STEP2 CS，是必须要考的内容。
综上，目前USMLE考试分为STEP1 and STEP2 CK， STEP2 CS，STEP3
通过上面的两个考试后，学生才有资格参加住院医师的申请，一般都要花上上千美金，申请50多个医院以上，才可能有面试机会，才好参加NRMP的3月份的 MATCH.足够优秀的申请人才会有机会进入有培训资格的.医院进行你住院医阶段的工作。需要说明的一点是，中国医学院毕业的学生是完全没有可能申请到像Harvard，Columbia，Yale这种顶尖医院的住院医师的，只能去一些三四流的医院。这时候需要申请人正确对待现实，避免巨大的心理落差造成的失落。
进入到住院医师的阶段之后，要经历3年痛苦的过程，然后是2年专科训练，一般在第二年住院时开始申请。经历5年的训练，就可以直接做专科医生了。但是在此期间薪水方面非常的窘迫了，美国住院医生的薪水比较固定，在各个医院的网站上都有注明，一般中部地区38，000到40，000，东部和加州稍为高一点，43，000左右。比POSTDOC略高，在美国是贫困线左右的，而且劳动强度很大。现在美国出台了专门保护住院医生的法规，比如每周工作不得超过 80小时，但是很多时候还是难以保证，尤其是对外国医生，需要更长时间完成工作。
所以艰苦程度就可想而知了，每年都会有大量的人被踢出局，你面临的不仅是医术，还包括英语，交际等诸多方面的挑战，如果你有足够的毅力，你是可以坚持下来的，但是一定要做好吃苦的准备，这和国内的优越条件和安逸的生活局面是完全不能比的。在住院医师期间你还必须要完成STEP3，连考2天，每天8小时，除去临床医学本身的考试外，还包括临床中的伦理、法律等相关内容。
考试通过以后，必须要接受ECFMG的学历认证，就是认证你在国内的临床医学的学习是否是被认可的，之后，你就能够拿到美国的BOARD CERTIFICATE，就是可以独立行医的医生了。在这一过程中，很多人都因为坚持不下来放弃了，所以其实参加这个考试的人很多，但是真正坚持下来的人非常少！即使是坚持下来了，可以独立行医了，也面临着很大的问题，原因在于种族，文化的不同，想要得到美国人的认可和信任是很难的，即使你有行医执照，但是美国人也未必愿意找你看病。
关于考试最好一次性通过，并且第一次就要考到95分以上，因为在申请住院医师的时候很看重你是否第一次通过是不是高分，如果分数很低，是不会有医院聘用的。医师执照考试有七年的期限，如果不能在规定时间内考出所有考试，完成三年的住院医生以前的努力就白费了。另外，在申请MATCH的时候医院也会看重你的年龄，因为毕竟住院医是个体力活儿，所以这对于中国申请者来说也是一个不大不小的考验，因为等待绿卡就是一个非常漫长的过程。 当然现在国内也是可以参加USMLE考试了，考点在北京，上海和广州，但是只可以参加STEP1和STEP 2 CK的考试，最重要的STEP 2 CS和STEP3必须在美国本土进行，所以即便你在国内完成了前两个考试，你如果拿到正式的身份去那边继续完成后面的考试，怎么能够顺利获得签证也是非常重要的问题，而且在国内的资源非常有限，能够考出来确实非常不容易。

1、办理社会安全卡
社会安全卡(social security card)在美国留学期间是非常重要的。留学生落地美国，带着行李进入学校后，首要事情就是申请社会安全卡，它能够开通电话服务和银行账户、信办理用卡、学校奖学金拨给都需要使用到它，根据学校的明确规定操作即可完成申请。
